SURMOUNT-MMO (cardiovascular outcomes in chronic weight management)

Trial: A randomized, double-blind, placebo-controlled cardiovascular outcomes trial of tirzepatide for the reduction of MACE in adults with obesity and established cardiovascular disease (NCT05556512).

SURPASS-CVOT (cardiovascular outcomes in type 2 diabetes)

Trial: A randomized, active-controlled cardiovascular outcomes trial of tirzepatide vs dulaglutide in adults with type 2 diabetes and high cardiovascular risk (NCT04255433).

Current cardiovascular evidence base

As of May 2026, tirzepatide has demonstrated weight loss, A1C reduction, blood pressure reduction, and improvements in lipid profile and inflammatory markers in the SURMOUNT and SURPASS programs — all surrogate cardiovascular risk markers. The definitive MACE-reduction evidence is pending the SURMOUNT-MMO and SURPASS-CVOT readouts. The most relevant approved cardiovascular label currently belongs to semaglutide: Wegovy is FDA-approved (March 2024) for reduction of cardiovascular events in non-diabetic adults with overweight/obesity + established CVD based on SELECT.

Questions readers ask

When are SURMOUNT-MMO results expected?

The page lists a late 2027 readout for SURMOUNT-MMO, a placebo-controlled trial in adults aged 40 or older with a BMI of 27 or more and established cardiovascular disease. Its primary endpoint is time to first MACE-3 event.

What is tirzepatide compared against in SURPASS-CVOT?

SURPASS-CVOT compares tirzepatide with dulaglutide (Trulicity) in adults with type 2 diabetes and established cardiovascular disease or multiple risk factors. Dulaglutide itself showed a MACE benefit in the REWIND trial published in the Lancet in 2019.

Which GLP-1 has a heart-protection label for people with obesity but no diabetes?

Only semaglutide 2.4 mg (Wegovy), FDA-approved in March 2024 for cutting cardiovascular events in that group based on SELECT. Tirzepatide has improved surrogate markers such as blood pressure and lipids, but the page treats definitive MACE evidence as pending.

Does tirzepatide produce more weight loss than semaglutide?

In SURMOUNT-1, tirzepatide 15 mg produced 20.9% mean weight loss at 72 weeks (treatment-regimen estimand), versus 14.9% at 68 weeks for semaglutide 2.4 mg in STEP 1 (treatment-policy estimand). These were separate trials, so the comparison is indirect.
